Is It Sadness or Depression? Signs It's Time to See a Psychiatrist

Feeling sad after a loss, a failure or a hard week is part of being human. It usually eases with time, rest and the support of people around us. Depression is different. It is a medical condition that changes how you think, feel and function — and it does not simply pass because you try harder. The good news: it is common, and it is very treatable.
Sadness vs depression: the key differences
| Ordinary sadness | Depression | |
|---|---|---|
| Cause | Usually linked to a clear event | May have no obvious reason |
| Duration | Hours to days, easing over time | Most of the day, nearly every day, for two weeks or more |
| Enjoyment | You can still enjoy things | Loss of interest in things you used to enjoy |
| Daily life | You keep functioning | Work, study, family life or self-care become hard |
| Self-view | "This is a hard time" | "I am worthless" or "things will never get better" |
Common signs of depression
Doctors look for a pattern lasting at least two weeks. You may notice:
- low mood, emptiness or tearfulness most of the day
- losing interest or pleasure in most activities
- sleeping much more or much less than usual
- changes in appetite or weight
- tiredness and low energy, even after rest
- difficulty concentrating or making decisions
- feeling guilty, worthless or like a burden
- moving or speaking more slowly, or feeling restless
- thoughts that life isn't worth living
Depression can also show up physically — headaches, body aches or stomach problems with no clear cause. In many families these physical symptoms are noticed first, while the low mood behind them goes unnamed.
"Just be strong" is not a treatment
In Pakistan, many people still hear that depression is a lack of faith, willpower or gratitude. It is none of these. Depression involves real changes in brain chemistry, sleep and stress hormones, and it can affect anyone — successful professionals, students, new mothers, elders. Seeking help is a sign of strength, not weakness, and a psychiatrist's consultation is confidential.
When should you see a psychiatrist?
Book an appointment if:
- symptoms have lasted more than two weeks
- low mood is affecting your work, studies, relationships or sleep
- you are using alcohol, sleeping pills or other substances to cope
- you have tried to manage on your own and it isn't improving
- a family member has noticed a change in you and is worried
Get help immediately if you have thoughts of harming yourself or ending your life. Go to the nearest emergency department or call Rescue 1122. You do not need to wait for an appointment.
How depression is treated
Treatment is personal, and most people improve with the right plan. Options include:
- Talking therapy, such as cognitive behavioural therapy (CBT)
- Medication, adjusted carefully to your needs and side effects
- Lifestyle support — sleep, activity, routine and social connection
- Specialist treatments such as ketamine therapy, for depression that hasn't responded to standard care
What happens at your first visit
Your psychiatrist will ask about your symptoms, sleep, physical health, family history and what you have already tried. You will leave with a clear explanation and a plan you agree with. At TLC you can be seen in the clinic in Johar Town, Lahore, or by video consultation from home.
